Prepaid Verification Voucher Information

Employers and related organizations can use this system to order primary source verification of individuals holding active AANPCB certification.

DO NOT USE THIS FORM FOR STATE BOARD OF NURSING NOTIFICATION.

New certifications may not appear until a few weeks after exams due to administrative processes. If verification is ordered before the certification process is complete, the verification letter will state that the individual is not certified.

Verification letters will be sent via email.
Voucher may be purchased by check or credit cards.

The AANPCB Verification Department offers the option for Business Customers who require closer tracking of their verification orders to purchase a PREPAID VERIFICATION VOUCHER. These vouchers are redeemable for up to 1 year from the date of purchase.

The minimum verification prepayment amount requested must be in increments of $40. The current minimum and maximum balance amounts are:

Minimum voucher purchase: 2 ($80.00)
Maximum voucher purchase: 625 ($25,000.00)

Vouchers expire one year from the date of purchase. Once an order is received and processed, unused vouchers are non-refundable.

Procedure

Download and complete the Prepaid Verification Voucher Order Form. Mail the form with credit card information or a check to AANPCB.
Once an order has been processed, a voucher number for each $40 of the total paid amount will be generated and emailed to the purchaser's email address.
To redeem a voucher, enter the nurse practitioner's last name in the 1st box below.
Enter the nurse practitioner's AANPCB certification number in the next box. The certification number must begin with an A (adult), F (family), A-G (adult-gerontology), or G (gerontologic). If the number you are entering does not begin with an A, F, AG, or G, it is not an AANPCB certification number.
Follow the directions on subsequent screens.
When the Billing Address Payment Method screen appears, verify the purchaser's billing address, and enter the voucher number for each verification ordered.
Submit the order. A receipt with a link to the AANPCB Verification of Certification Letter will appear on the screen after the order is submitted. The Verification of Certification letter will also be sent as an email attachment.

Information accessed through this process is a true and accurate reflection of our records, which are derived from primary source data.
